Apprenticeship Agreement Form


Signing the apprentice: the apprentice must sign the agreement which stipulates that they have read the terms of the agreement and that they accept. Recognition for previous experiences: credits can be granted, but no more than 50% of the duration of the apprenticeship. Credit experience may be granted by the program sponsor, but must be approved by DAS All apprenticeship contracts must be submitted by e-mail within 30 days of the start of the training and must be accompanied by school information, a passport image and an online payment worth USD 35 via the Online Payment System of the Department of Laboratory Standards: www.mass.gov/service-details/department-of-labor-standards-online-payment. School law: when the apprentice signs the apprenticeship contract, he accepts the conditions on the back of the agreement and gives access to all the corresponding teaching materials. Agreements can be presented apprenticeship@mass.gov. Start date: the date on which the apprentice must have signed the contract and received the DAS within 30 days of that date. An apprenticeship contract must be signed at the beginning of the apprenticeship. It is used to validate individual employment agreements between the apprentice and the employer. Once all the information has been completed, it must be signed and forwarded to DAS for approval before it can be used as an exported document. Union signature: if the sponsor of the programme is part of a union, the union must also sign an agreement. The apprentice undertakes to do the work and take the corresponding courses.

Aggregate statistical information: The apprentice must provide DAS with his social security number, email address, date of birth, home phone number and check the corresponding boxes for gender, race, education and veteran status. Address: The apprentice`s full residence address (number, street/post office box, city, land and postcode) Parent/guardian signature: legal guardian`s signature, if necessary. Trade: an industry for which the apprentice recorded a previous salary share: the amount of salary received by the apprentice in a previous job. Confirm receipt of das manual: Download a learning data manual. Apprentices are in the original school. Training period: the length of time it takes to obtain the status of a traveller in a particular trade. Periods: Each period is 1,000 hours and corresponds to the steps necessary to achieve traveler status. (i.e.: 8 steps x 1000 hours per program - 8000 hours) Steps: Each step corresponds to a certain number of hours to complete the next step.
